How your health certificate is signed
The website fills in the producer and cheese details you provide. An official veterinarian, or a veterinarian authorised by the competent authority, checks those details, completes the veterinary section, then signs and stamps the certificate.
If the entry form told you no health certificate is needed, none of this applies to you.
Before your appointment. Print the certificate and take it to your appointment exactly as it is. Do not write anything on it yourself. Boxes I.2, I.2a, I.3 and I.4 are official boxes: leave them blank. Your veterinarian knows which of them their authority uses and which stay empty. The veterinarian completes Part II, then signs and stamps in blue wet ink. Any transport boxes left empty are empty on purpose. A company signature does not replace the veterinarian's.
Your official or authorised veterinarian must sign the original certificate in blue wet ink and apply their official stamp.
Bilingual certificate (English and Spanish, or Portuguese and Spanish): every mark must appear identically in both language versions. Same strikes, same initials, same stamps, same entries.
The paper signed and stamped by the official or authorised veterinarian travels with the consignment; a copy or scan is not enough at the border.
Place the original documents required for your shipment in an envelope securely attached to the outside of the box, so they can be checked without opening it. Keep your My Guild delivery label clearly visible.
Optional spare set
If possible, put a second set of original documents in the same envelope. Your consolidation point can keep the spare set separately as a backup.
